How to obtain a copy of a Birth Record
Application for Birth Certificate
The Town Clerk's Office has birth certificates on file for persons born in the Town Columbia. Only the person named on the birth certificate or the parents of the person named on the birth certificate are eligble to receive a copy. A spouse, child or other person may only obtain a copy of a birth certificate with a NYS Court Order. The applicant must be 18 years of age or older to receive a copy of his/her birth certificate. A person who wishes to obtain a certified copy of his/her birth certificate may come to the Town Clerk's office during office hours, with the appropriate identification and fee or it may be requested by mail. If you are requesting your birth certificate by mail, you must complete the application which you will find attached above, include a photocopy of one of the following:
- State issued driver's license
- State issued non-driver's identification card
- US military issued photo identification card
- Passport
If you do not have photo ID, copies of two (2) utility bills showing your name or two (2) letters from a government agency dated within the last six (6) months or a combination of both will suffice. A certified check or money order for the appropriate amount made payable to the Columbia Town Clerk must be included. The cost is $10 per copy. Once the request is received it is processed and mailed the same day. Failure to include the necessary information will result in rejection of your application.
Please note when applying for a copy of your birth certficate via mail and your driver's license or other photo id does not reflect the address to which you want the certificate mailed, you must have your signature notarized.
